Day Habilitation (DHI or DHG)
Services provided outside of the Participant’s home that support learning and assistance. Community based activities are intended to build relationships and natural supports.
PARTICIPANT ASSISTANCE AND CARE (PAC)
Provided to allow participants (individuals) with intellectual/developmental disabilities to remain and live successfully in their own homes and participate in their communities.
RESIDENTIAL HABILITATION SERVICES
Provide up to a full day (24-hour basis) of services and/or supports which are designed to ensure the health, safety and welfare of the participant, and assist in the acquisition, improvement, and retention of skills necessary to support participants to live successfully in their own homes
RESPITE (RSPO)
Services provided to participants unable to care for themselves that are furnished on a short-term basis in order to provide temporary relief to those unpaid persons normally providing care
TRANSPORTATION
Transportation is provided in conjunction with other services, to allow the individual to become integrated in his/her community
WELLNESS COORDINATION
The development, maintenance and routine monitoring of the waiver participant’s Wellness Coordination Plan and the medical services required to manage his/her health care needs
Extended Services (EXTS)
Services and supports that enable a participant who is paid at or above the federal minimum wage to maintain employment in a competitive community employment setting
